The Change of Condition (CoC) mobile application, built by Y Point Analytics, in partnership with Minuteman Senior Services, facilitates at-home delivery of meals to seniors by driver volunteers who report back delivery status of meals and any perceivable change in health condition of the meal recipient.
Our award-winning, feature rich application provides the following features to drivers:
• Route preview on a map before beginning your delivery.
• Optimized route, which allows fastest delivery while traveling the shortest distance.
• Point to point directions using Google Maps.
• Mark meals as delivered or Undelivered
• Provide instant feedback on customer health condition.
• Provide real-time notification on meal-delivery and change of health condition.
• Ability to view special instructions on delivery to a customer.
• Ability to view history of all completed deliveries for the day.
• Ability to redeliver an undelivered meal.
Once an authorized account has been setup by your management, the driver/volunteer can download the app and begin delivery on the route assigned. An optimized sequence of deliveries is listed on the delivery list, with the completed deliveries/non-deliveries logged in the history tab.
